Submit an Incident Report (iReport)
Employers or occupiers can submit a work-related incident report. Doctors can notify MOM for employees who have Occupational Diseases. You can also amend, purchase or download iReport through WSH Incident Reporting. Find out more on what else you can do with this eService.

Business users
Occupier, platform operator or insurer, logging in with your organisation's Singpass or CorpPass.
- FOR INDIVIDUAL USERS
Individual users
Platform worker, treating doctor, injured employee, next-of-kin or legal representative, logging in with your own Singpass.
Are you an employer?
In the live service every employer journey — a work accident, an occupational disease, or an accident involving an employee who was also performing platform work — is filed through the myMOM Portal, which is also where you select your organisation's UEN/Entity ID. That portal is a separate project and is not part of this e-service, so sign in through Business users above with your organisation's CorpPass and choose Employer as your role.
